The midfield is a three-man unit centered around a single pivot. Tommi Conte sits deep to shield the defense and intercept passes. Bernardi and Di mauro operate as the two central midfielders who drive forward with the ball and connect the defense to the attack. These players are tasked to press in a mid-block and win the second ball. Such a group is essential to progress through the thirds and recycle possession when the attack stalls.

The attack features two forwards working as a partnership. Bertocchi and Cirillo play to press from the front and make runs in behind the defensive line. They often combine in tight spaces to find the feet of the strikers or play a one-two to break the line. To provide width, Uras and Fiche push up to act as wing backs. They look to get to the byline, whip it in, or cut it back for the central players.

A primary strength for Napoli is the ability to create numerical superiority in the middle of the pitch. By having three central players, they can often outnumber the opposition's midfield. Additionally, the formation allows for wide overloads when Uras and Fiche push high to overlap the midfielders. This creates space for Bertocchi and Cirillo to find pockets of room between the lines.
